New Patient Registration
If you live within the catchment area for Castle Healthcare Practice and would like to register, please click here to complete the online registration form.
Please note that you must bring proof of your address (utility bill, bank statement etc) to reception to complete your registration.
If you would like a paper copy of the registration form, please ask at our reception desk.
Please note – If you are taking any regular medication you may need an appointment to speak a GP before a repeat prescription can be issued. Please ensure you have a sufficient supply from your current Practice before registering.
New Patients – We are required to provide a named and accountable GP for all patients. Yours is the Registered and Usual GP automatically allocated when registering at Castle Healthcare Practice. If you require more information please ask at reception. This may not necessarily be the GP that you choose to see.
